The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region. The IUIH West Moreton Mental Health Hub aims to provide comprehensive assessment treatment and assertive outreach support to clients presenting with moderate to severe mental health needs. Working in a unique multidisciplinary care team as an extension of the IUIH comprehensive model of care, along with integration with the West Moreton mental Health teams to provide appropriate and responsive care to clients would otherwise not access support or require inpatient care. About the Role? As a staying deadly support worker, you will take lead from the care coordinator and multidisciplinary care team to holistically walk alongside our clients. Providing practical psychosocial supports in the following domains: Promoting client independence by developing and working towards individual goals. Working to remove barriers so that clients have improved access to services within the community that enhance individuals' emotional well-being. Working from a client and family centred perspective with consideration given to cultural, emotional, and physical needs. Working relationships with internal and external services to facilitate referral pathways and clear communication. Your day to day duties will include: Deliver support in a manner which ensures meaningful engagement of the person in all decision making, with a bias toward building individual capability to participate in and lead the coordination of their own supports. Ensure engagement of natural supports and networks to compliment funded services and assist in supported decision making where relevant and consistent with customer preferences. Assist in the coordination and delivery of services for Staying Deadly Hub clients, in accordance with a case management plan and/or team care arrangement. Work flexibly in response to client and family needs Conduct home visits or community visits to support ongoing client recovery. About You? Current C Class Drivers Licence (QLD) Current Criminal History Check Valid Blue Card Desirable, but not mandatory: Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities What Can we Offer?
