Position summary:
The Skills Facilitator is responsible for assisting clients of West Coast Supports to maintain and improve their capacity with life skills, to promote an independent life.
The Skills Facilitator is expected to deliver a person-centred approach to support, and provide innovative methods to health and disability problem solving.
Key responsibilities:
• Support skill development and capacity building in domestic assistance activities in a client’s home to enable them to remain safe and independent in the community. These tasks may include supporting individuals with personal care needs such as mealtime management, hygiene practices, and household maintenance.
• Observe clients health and wellbeing and report/feedback as necessary to line manager
• Build the resilience and self determination skills of clients, their carers, and family members to advocate for their needs within the mainstream government system, and broader community
• Maintain a high level of confidentiality.
• Informing your leader of innovative and evidence based ways of care, including methods of service delivery that are industry leading, whilst implementing these approaches in a safe way

Service provision tasks may include:
Supporting and facilitating skill development in assistance with self-care activities such as:
• enabling choice and control related to outfits and assistance with dressing
• assistance with capacity building related to hygiene and grooming practices
• assistance with mealtime management and informing clients of wellbeing related to diet
• assistance with client self- administration of medicine and development of understanding medication
• promotion of self-management options in daily activities, including plan management and self-advocacy during goal and funds planning

Leadership and Governance:
• To actively initiate and facilitate the development of strong and effective goal achievement outcomes with clients, their families, and carers.
• Ensure compliance with all legislative requirements associated with the role
• Model, promote and adhere to West Coast Supports policies and procedures, including our mission of delivering health and disability services in an innovative and influential manner
• Promote an inclusive culture that enables team members to perform at their best and contribute to the success of West Coast Supports as a market leading organisation
• Ensure all communication within the organisation is transparent and fosters open dialogue that enables all stakeholders the opportunity to contribute, be heard and receive feedback.

• Keeps up to date with mandatory training requirements for the role
• Maintains practice within framework established by legislation, national and state policy
• Recognises areas of strength and areas for development
• Shares knowledge willingly

Information management:
• Communicates accurate and relevant information to appropriate people and in line with the organisational policy.
• Is able to access, send and receive email and utilise other Microsoft products relevant to the role
• Uses social media responsibly and respectfully at all times
• Complies with legislative requirements related to documentation and information pertaining to clients, team members and the operations for all West Coast Supports.
• Utilise information technology as appropriate in the performance of the role.
ESSENTIAL:
• Ability to read, comprehend and adhere to support and business plans
• Proven ability to communicate effectively with clients, their relatives, carers or significant others
• Current First Aid Certificate
• Current Western Australia Driver’s License
• NDIS worker screening check
• WWC check (if providing services for anyone under the age of 18)
