Your part in our UnitingCare team Reporting to the Disability Services Manager, the Service Co-ordinator is responsible for executing on the operational plan for the planning, delivery, workforce management and financial sustainability of all NDIS service provision relating to the provision of core supports and capacity building to clients. Through the development, supervision and mentoring of lead practitioners across all arrangements, the Service Coordinator ensures that outcomes for individual clients are met according to individual plans and budgets. They participate in the in the development of community opportunities and increasing services according to individual client preferences, development of new arrangements, increasing services and client retention and have responsibility to oversee quality practice of service models in a cost-effective manner. Participates in and manages business development and operational plans for delivery of all core supports and capacity building to UCQ customers; Foster a values-driven, safe employment environment that attracts, engages and retains skilled employees and volunteers Coach and develop direct reports daily, ensuring development goals are linked to business objectives and improved performance Works collaboratively with other service leaders to provide a coherent and successful client journey through UCQ service provision; Manages the relevant cost centres in its cluster including planning for, increasing and delivering of services in a financially sustainable manner; Mentors, develops and enables Lead Practitioners to operate their arrangements autonomously including to provide exceptional services, supervision, rostering and engagement of all direct support employees; Operates as the escalation point for issues arising in arrangements that are unable to be managed by Lead Practitioners and assigns additional assistance such as by Complex Service Advisors or outside consultants where required in order to enable client outcomes; Works with Service Design Manager to schedule Lead Practitioner resources to effectively support planning of client supports; Works effectively with the Reporting & Capability Coordinator and/or the cluster support team to provide operational data for the effective reporting and monitoring of service provision, workforce metrics and quality in order to inform the continuous improvement agenda of the service; Provide effective performance management and compliance with relevant legislation, standards, Uniting Church and UCQ Board Policies What makes a difference for us? Health & Community Services related Degree or Qualification: Health & Community Services industry experience Exceptional understanding of the NDIS and its applications in a client centric service model Knowledge of legislation underpinning the provision of services within the disability sector and the key issues affecting quality of life for people with a disability Experience operating at a management level including managing financial imperatives and scheduling Knowledge of relevant legislation, policy and licensing requirements associated with the Disability sector Sound understanding of the application of the principles of family or self-governance, personal autonomy, choice and control and intentional safeguarding for our clients Demonstrated ability to provide advice, support, coaching, mentoring and training to managers and staff Ability to think creatively and critically and design plans that deliver personal outcomes while being practical and financially sustainable for UCQ; and Ability to consider risk and where specialist service provision is required and able to effectively collaborate with other managers and specialists to achieve outcomes NDIS Worker Screening Clearance and Blue Card First Aid and CPR Applications will be reviewed as they are received, and therefore may result in the role closing early if the right applicant is found.
