About the Role: Manage complex community patients with chronic health and social conditions, providing high-quality social work services and biopsychosocial assessments. Conduct risk assessments related to capacity, self-determination, domestic violence, and elder abuse, facilitating Advance Care Planning and substitute decision-making discussions. Advocate for patients and carers, coordinating referrals to services such as My Aged Care, NDIS, and NCAT Guardianship. Collaborate with multidisciplinary teams and external agencies to ensure comprehensive, culturally sensitive care and effective discharge planning. Provide clinical supervision, education, and placements for social work students while adhering to professional ethics and standards. Lead quality improvement initiatives and represent the social work department on internal and external committees. About You: Qualified Social Worker with eligibility for AASW membership and strong clinical experience in aged care and health social work. Skilled in psychosocial assessment, crisis intervention, advocacy, and complex discharge planning within diverse clinical frameworks. Experienced in managing complex social issues affecting patients, families, and carers across aged care, rehabilitation, disability, and residential settings.
