About the Role
We are seeking an experienced Registered Nurse (RN) who is looking for a flexible, meaningful role supporting older people to live safely and independently at home.
This role is ideal for a semi-retired nurse or an RN seeking reduced hours, autonomy, and community-based work without shift pressure. You will provide clinical oversight, assessment, training and support to care workers as well as clients receiving Support at Home services across Sydney.
Hours are flexible and can be arranged to suit your availability.

Key Responsibilities
• Conduct in-home and/or virtual clinical assessments for new and existing clients
• Develop, review, and update care plans in collaboration with participants and registered supporters
• Monitor and manage clinical risks, including:
• Medication management
• Wound care oversight
• Falls risk
• Deterioration and escalation
• Provide clinical guidance, compulsory annual training and support to care workers and support staff
• Identify and escalate changes in participant condition appropriately
• Support compliance with the Aged Care Strengthened Quality Standards
• Maintain clear, concise clinical documentation
• Liaise with GPs, allied health professionals, and external providers as required
What We’re Looking For
• Current AHPRA registration as a Registered Nurse
• Minimum 3–5 years post-registration experience (aged care, community, or general nursing preferred)
• Sound clinical judgement and a calm, practical approach
• Strong communication skills and confidence working independently
• Understanding of aged care, community care, or home-based services
• Basic computer skills for documentation and care systems
• Valid driver’s licence and access to a reliable vehicle
