About the role You will join a dynamic Home Care allied health team to deliver high-quality Physiotherapy services to HammondCare clients in the Inner West Sydney Community. This is an exciting 12 month fixed term full-time role with a broad caseload. Whilst you’ll have the autonomy to structure your own day to meet key deliverables, you will also be supported by an experienced interdisciplinary (IDT) team. Together you will deliver client-centred, evidence-based care in an environment that promotes continuous quality improvement consistent with the values and ethics of HammondCare and Australian Health Practitioner Regulation Agency (AHPRA). What will your key responsibilities look like? Provision of a high-quality Physiotherapy service to HammondCare's Support at Home clients including individual 1:1 home visits and group activities. Assess and provide in-home rehabilitation programs to improve functional independence and quality of life. Collaborate closely with our skilled IDT to assess clients’ functional care needs, set care goals, and provide treatment and interventions to deliver the best possible client outcomes. Participate as a member of the HammondCare team in the application of the Mission and values to the pursuit of its objectives in a person-centred, continuous quality improvement work environment. Participate in quality improvement projects and continuing professional development. Provide clinical supervision for students, if appropriate. About you Tertiary qualified physiotherapist, with current AHPRA registration and practical experience or sound knowledge of physiotherapy in a Rehabilitation or Aged care setting. This can include inpatient/outpatient, community settings or both. You’re a strong communicator, and able to plan and manage your time effectively. Enjoy working independently and as part of a multi-disciplinary team.
