We are seeking a skilled freelancer to improve a user-friendly form using JotForm based on our existing risk-based IR diagnostics tool developed in Excel. A version has been developed and now needs to be improved to give it a professional look and feel. The project will improve the Jotform, complete with clear instructions for users. The ideal candidate will have experience with both JotForm and Excel, as the data will flow to Power BI. The input form poses a number of questions to the user, requiring two answers, and provides a calculation to the user based on the scores entered. The freelancer must be experienced in these tools including using formulas in Jotform - Approx 39 line items for the questions.
