Fat Loss Foundations Agreement (Online Coaching Program) – includes program terms, liability waiver, and payment clauses. I need these documents converted into professional, user-friendly fillable PDFs that clients can complete and sign digitally. Key Requirements: Add fillable fields for: Client details (name, address, phone, email, DOB, emergency contact). Checkboxes (e.g., Yes/No health questions, goal selection, media consent opt-out). Signature and date fields for both client and trainer. Larger text boxes for open responses (e.g., health notes, goals). Ensure layout stays clean and professional. Keep branding intact (logos and headings already included in PDFs). Final PDFs must be compatible with Adobe Acrobat, Preview (Mac), and common e-sign tools.
