About the Position
As a Community Worker at Bluebird Care for Disability Services, you will play a pivotal role in connecting NDIS participants with vital resources and support networks, assessing community needs and coordinating programs that promote social inclusion and well-being for people with disabilities. This position encompasses facilitating client interviews, developing tailored educational and support initiatives, and liaising with external agencies to ensure seamless service delivery, all while upholding the company's commitment to flexible, client-centred care. Key Responsibilities
Assess clients' needs through interviews and evaluations, planning, developing, and implementing educational, training, and support programs tailored to individuals with disabilities.
Monitor and report on client progress and program effectiveness, making adjustments to ensure optimal outcomes and compliance with NDIS standards.
Refer clients to appropriate agencies and services for additional support, such as health, welfare, or housing resources.
Liaise with community groups, welfare agencies, government bodies, and private businesses to promote Bluebird's services and address broader community issues.
Organise and facilitate community events, activities, and participation programs to combat isolation and foster social connections for NDIS participants.
Assist vulnerable groups, including those with disabilities, in solving social, emotional, and practical challenges through targeted interventions and advocacy.
Requirements
Australian citizens and permanent residents are encouraged to apply.
A relevant Diploma in Community Services, Welfare Studies, or a related field.
At least 3 years of experience in community support, disability services, or welfare roles.
Strong technical skills, including proficiency in case management software, data entry for reporting, and familiarity with NDIS funding and compliance requirements.
Excellent interpersonal skills, such as empathy, active listening, cultural sensitivity, and the ability to build trusting relationships with diverse clients and stakeholders.
