Position Summary
> GNG Healthcare Group is seeking a compassionate and experienced Community Worker to facilitate community development initiatives and collective solutions that address issues, needs and problems associated with recreational, health, housing, employment and other welfare matters within the communities we serve. This role is crucial in helping individuals maintain independence and improve their quality of life by connecting them with essential services and resources, aligning with GNG Healthcare's commitment to personalised and accessible care. The Community Worker will focus on advocacy, resource linkage and program development, operating at a skill level commensurate with a bachelor’s degree or equivalent.
Key Responsibilities include:
• Community Needs & Engagement
> Identify the needs and interests of individuals and groups within the NDIS community, especially those at risk of social isolation or disadvantage
> Engage with participants, families, carers and stakeholders to understand barriers to community participation and service access
> Facilitate opportunities for NDIS participants to become more involved in their local communities
• Liaison and Collaboration
> Build and maintain strong relationships with local community groups, service providers, government agencies and private organisations
> Promote collaborative service delivery approaches to meet shared goals across sectors (e.g., housing, education, employment, health)
> Represent GNG Healthcare Group at community forums, meetings and inter-agency networks
• Program Planning & Delivery
> Plan, develop and implement community-based education, engagement and capacity-building programs tailored to the needs of NDIS participants
> Organise and facilitate group workshops, recreational activities, training events and information sessions that promote life skills, independence and wellbeing
> Support the delivery of culturally safe, inclusive and accessible community programs
• Information & Referral
> Promote awareness of community supports, rights and available services, helping participants make informed choices
> Provide basic assistance and referrals to appropriate support services (e.g., employment programs, housing services, healthcare, education).
> Support participants in navigating community systems and services in alignment with their NDIS goals
• Reporting & Administration
> Maintain accurate and confidential records of community activities, participant engagement and program outcomes.
> Contribute to internal reports, funding submissions, evaluation processes and program reviews
> Ensure documentation meets compliance and quality standards in line with organisational policies
Qualifications and Experience:
> Bachelor's degree or equivalent in healthcare coordination or community services
> Minimum of 1 year experience in community services, focusing on facilitating community development, resource coordination, and addressing welfare matters
> Current and Valid Western Australia Driver's Licence
> Current First Aid and CPR certification
> NDIS Worker Screening Check
> Strong communication and interpersonal skills, with the ability to engage effectively with diverse stakeholders and clients
> Ability to work independently and as part of a multi-disciplinary team
Desirable Additional Experience
> Experience working with culturally and linguistically diverse (CALD) communities
> Training in mental health first aid or psychological support
> Background in case management, support coordination, or allied health services
> Familiarity with the NDIS scheme processes and person-centred planning and supports
