About the Position:
The Community Worker role at V & C Nursing Pty Ltd involves providing hands-on support to clients with disabilities, elderly individuals, or those requiring complex care, helping them achieve independence and engage actively in their communities. This full-time position focuses on delivering personalized care in clients’ homes or community settings, ensuring their physical, emotional, and social needs are met with compassion and professionalism.
Key Responsibilities:
Assist clients with daily living activities, including personal care, mobility, and household tasks.
Support clients in community participation, such as attending social events or appointments.
Implement individualized care plans to promote independence and well-being.
Monitor and report on clients’ progress, communicating effectively with families and healthcare professionals.
Provide emotional support and companionship to enhance clients’ quality of life.
Requirements:
Australian citizens and permanent residents are encouraged to apply.
Qualifications: Diploma in Community Services, Disability Support, or a related field.
Experience: Minimum of 1 year of experience in community care, disability support, or aged care.
Strong interpersonal and communication skills to build trust with clients and families.
Empathy and compassion for working with diverse client groups, including those with disabilities or complex needs.
Ability to work independently and as part of a team in dynamic environments.
Basic knowledge of NDIS frameworks and care plan implementation.
Valid driver’s license and reliable transport for community-based work.
