Horizon Community Service is a trusted NDIS-registered provider, dedicated to empowering individuals with disabilities to live independently, achieve their goals, and actively participate in the community. Based in Eight Mile Plains, QLD, we offer a wide range of person-centred services including in-home care, community access, life skills training, and supported independent living. We are currently seeking a motivated and caring Community Support Worker to join our supportive team. Role: Community Support Worker

Key Responsibilities
In this role, you will:
• Meet with clients to understand their needs and challenges, developing tailored support and care plans.
• Assist individuals and families with social, emotional, and financial wellbeing, offering guidance and practical solutions.
• Deliver day-to-day support, including personal care, life skills development, and participation in community activities.
• Monitor client progress, maintain accurate records, and report outcomes to ensure services meet goals and compliance standards.
• Refer clients to appropriate health, housing, employment, or training services when additional support is required.
• Liaise with community groups, healthcare providers, government agencies, and private organisations to advocate for client needs and promote access to available resources.
• Provide support and education in group housing or supported living environments, ensuring safety, dignity, and independence.
• Prepare reports, funding submissions, and documentation for government or agency requirements.
• Promote awareness of community services and encourage inclusion through events and initiatives.
• Uphold Horizon’s values by providing respectful, culturally sensitive, and client-centred care.

Qualifications & Experience
• Minimum AQF Associate Degree, Advanced Diploma, or Diploma in Community Services, Social Work, Disability, or a related field.
• At least 1 year of experience in a similar role within disability, aged care, or community support services.
• Sound knowledge of NDIS frameworks, community resources, and support services.
• Strong written and verbal communication skills with the ability to build trust and rapport with clients and families.
