Horizon Community Service is a trusted NDIS-registered provider, dedicated to empowering individuals with disabilities to live independently, achieve their goals, and actively participate in the community. Based in Eight Mile Plains, QLD, we offer a wide range of person-centred services including in-home care, community access, life skills training, and supported independent living. Our mission is to provide compassionate, professional, and reliable support that enhances the quality of life for every client. We are currently seeking a motivated and caring Community Support Worker to join our supportive team. Role: Community Support Worker

Key Responsibilities
In this role, you will:
• Meet with clients to understand their needs and challenges, developing tailored support and care plans.
• Assist individuals and families with social, emotional, and financial wellbeing, offering guidance and practical solutions.
• Deliver day-to-day support, including personal care, life skills development, and participation in community activities.
• Monitor client progress, maintain accurate records, and report outcomes to ensure services meet goals and compliance standards.
• Refer clients to appropriate health, housing, employment, or training services when additional support is required.
• Liaise with community groups, healthcare providers, government agencies, and private organisations to advocate for client needs and promote access to available resources.
• Provide support and education in group housing or supported living environments, ensuring safety, dignity, and independence.
• Prepare reports, funding submissions, and documentation for government or agency requirements.
• Promote awareness of community services and encourage inclusion through events and initiatives.
• Uphold Horizon’s values by providing respectful, culturally sensitive, and client-centred care.

Qualifications & Experience
• Minimum AQF Associate Degree, Advanced Diploma, or Diploma in Community Services, Social Work, Disability, or a related field.
• At least 1 year of experience in a similar role within disability, aged care, or community support services.
• Sound knowledge of NDIS frameworks, community resources, and support services.
• Strong written and verbal communication skills with the ability to build trust and rapport with clients and families.
