The Opportunity The role of the Care Team Leader is to lead the team of Community Care staff who provide direct care to our clients. This is Permanent Full-Time position working Monday – Friday, located in the Northern Sydney Home Care team. About the role The role ensures that staff have a good understanding of individual client needs and preferences and can provide services tailored to the needs of individual clients, delivered within budget and service expectations. Responsibilities include; Leading, supporting, and mentoring their team, and works in partnership and consultation with the Care Manager, Clinical Care Manager, Client Liaison Manager, and Regional Area Manager, where required Manage the recruitment of new Care Workers and ensure all working validations, education, development plans and resources are available What do you need to apply? Certificate IV in Aged Care (Desirable), HR or similar degree & a minimum of 2 years relevant industry experience. Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme. Performance coaching experience and management of a team while operating in a multi-site environment A current driver’s licence and safe driving record and willingness to travel between HammondCare’s clients, sites, and services Strong organisational, administrative and computer skills Empathy and patience.
