Project Overview I need an experienced WordPress/Fluent Forms developer to build a new multi-section health questionnaire on my Elementor site. This is an advanced form project that goes beyond simple surveys. The form must: • Be built in Fluent Forms Pro • Include ~5 sections (Biotypes, Thyroid & Digestion, Glucose Regulation, Gluten Sensitivity, Lifestyle). • Use scenario-based multiple choice in some sections (each option maps directly to one or more categories). • Use 0–3 severity scoring for other sections (e.g., digestion, glucose regulation). • Display results as bar graphs (client-friendly labels). • Integrate with Google Sheets for basic data capture. • Integrate with Google Cloud BigQuery + Looker Studio for structured storage and reporting. ________________________________________ Reference Material Here is my current live questionnaire: 👉 https://balansoul.com/mood-appraisal-questions/ To know exactly what I’m looking for, please feel free to fill out this questionnaire. While the questions in the new project will be different, the layout and functionality should be the same. When you complete it, you’ll see: • Results displayed as bar graphs, • An email summary with results, • A downloadable results file, • And the ability to click into each results section/bar to see which questions contributed to that score. This is the exact type of results delivery I want replicated in the new form. However, for our new ‘Biotypes’ questionnaire, the questions are different and not based on ….0-3 severity scoring. Form shell & content load • Build the questionnaire form with ~5 sections. • Add all questions with correct field types. • Apply basic conditional logic (e.g., women-only questions, skip logic). 2. Scoring logic • Implement scenario-based mapping (answers → one or more categories). • Implement 0–3 severity scoring for functional sections. • Create hidden totals per category. • Provide a Score Map spreadsheet mapping each option to its category totals. 3. Results page & outputs • Configure results screen to display bar graphs (Fluent/Ninja Charts or Looker embed). • Add client-friendly labels and short explanatory text. • Configure confirmation emails with numeric totals + chart/PDF attachment. 4. Integrations & analytics • Connect form submissions to Google Sheets. • Configure Google BigQuery dataset for structured storage. • Link BigQuery → Looker Studio dashboards for advanced reporting. • Provide admin cheat sheet (PDF/Doc + Loom walkthrough). ________________________________________ Acceptance Criteria • Scenario questions correctly increment mapped categories (multi-category possible). • Severity-style sections calculate correctly (0–3). • Graphs render totals cleanly. • Results emailed with summary. • Google Sheets receives all fields + totals. • BigQuery dataset + Looker Studio dashboards functional. ________________________________________ Freelancer Requirements (Mandatory) • Proven experience with WordPress + Fluent Forms. • Must have built multi-category scoring forms/quizzes. • Must have set up Google Sheets, Google Cloud BigQuery, and Looker Studio integrations before. • Must provide at least one example of a Fluent Forms → BigQuery → Looker Studio pipeline they have built. • Must advise whether the reporting could be made more stable with a simpler setup (e.g., Fluent Forms → Google Sheets + embedded charts) instead of BigQuery + Looker Studio. • Must provide examples of previous work demonstrating these skills. Nice-to-have: • Experience with health/psychology questionnaires. • Data analysis/reporting background. ________________________________________ How to Apply Please provide: 1.
