About the opportunity The Commonwealth Home Support Program (CHSP) Occupational Therapy (OT) Team provides a community-based service for elderly individuals living at home and having difficulties with daily activities. We aim to assist our clients to participate in everyday activities, maximise their independence, safety and wellbeing. The CHSP AHA will work closely with the OT team to support the delivery of recommendations to optimise or maintain the client’s participation and independence in functional activities and occupations. The AHA will carry out delegated 1:1 follow up, within the community and the clients home; education as well as administrative tasks associated with supporting the OT team. This support ensures effective and efficient service delivery for clients and therapists. You will be expected to provide support to clients who live in the Western Metro region - work vehicles will be available for use. To thrive in this role, you will have: AHA certificate IV or equivalent. A valid and current unrestricted driver’s license. Demonstrated understanding and experience of aged care issues, the aged care service system (My Aged Care) and empathy with older adults is essential. The ability to work autonomously, make decisions, organise own workload and monitor own performance to achieve predetermined targets. A compassionate, flexible and positive approach to effective client care. The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment. You will also be required to provide evidence of, or in the process of obtaining: A current Police Record Check and Working with Children Check.
